The NDDOT Internet Utility permitting process is as follows: or where you are requesting to place utility on NDDOT right of way.

New Utility Permit Fees ( 6-15-06)*

1. Find the District contact in the location where the permit is being requested and call to request the appropriate number of Risk Management Appendix (RMA’s) forms to go with each permit form. To find out which district to contact, see map and Utility Permit Contact List below.

2. Fill out SFN 7995 Utility Occupancy Application and Permit form, (Rev. 05-2004)* (you will need Adobe reader to access this document.
In the event that the message “There was a problem reading this document (109)” Click on OK as many times as you need and the document should open) print the completed form, sign it, attach your certificate of insurance form, Risk Management Appendix form, (from district) and any related drawings, mail to NDDOT district office for approval. After approved, NDDOT District office will send utility company a copy.
